Cadre Reporting Form

The purpose of this ongoing data collection tool is to examine at two points during the year, and then annually, the level to which our transition planning and services have produced real results with students.

Most of you are working on several of these activities and dimensions, so please do collect and share your data in as many categories as you can.

We recommend you print out this tool a few weeks before it is due, engage your colleagues in helping you identify students (and numbers) who fit in these categories, and then go online to the nmtransition.org website to enter your data. (http://www.nmtransition.org/cadrereport)

Your data reports during the year should reflect the school year's efforts to that point SINCE THE LAST REPORT OR THE START OF THE SCHOOL YEAR.

Your annual data report should compile the information for the entire school year.

Be sure to keep a copy for yourself. We know that you will be able to use these data in a variety of ways, as will your principal and special education director. Most importantly, you will find these helpful to guide you in program and curricular revisions that are needed to improve student outcomes.
