Statewide Transition Participants and Services Project

This study, funded by the Developmental Disabilities Planning Council, will complete its work in September, to identify (a) services and supports provided to youth with disabilities as they make transitions from school to adult activities and (b) gaps in those services and supports. Five major components comprise this study: (1) analysis of agency roles and responsibilities, (2) analysis of reports and studies that identify the outcomes of those agency efforts, (3) analysis of focus groups with consumers on issues requiring more depthful information, (4) analysis of existing databases, leading to creation of a potentially shared database, and (5) recommendations.
