Who should attend?
For best results, come as a team – including transition specialists, special education directors and teachers, school administrators, parents, youth, adult agency partners, and other transition team members.
Purpose:
To assist local transition teams to improve their youths’/young adults’ outcomes in line with these State Performance Plan Indicators:
Ind. 1 – Graduation rates (on the right graduation options)
Ind. 2 – Dropout rates
Ind. 13 – Quality, comprehensive transition planning in the IEP
Ind. 14 – Post-school outcomes (working, learning, living)
