<p>For best results, come as a team – including transition specialists, special education directors and teachers, school administrators, parents, youth, adult agency partners, and other transition team members.</p>
<h3>Purpose:</h3>
<p>To assist local transition teams to improve their youths’/young adults’ outcomes in line with these State Performance Plan Indicators:<br />
Ind. 1 &#8211; Graduation rates (on the right graduation options)<br />
Ind. 2 &#8211; Dropout rates<br />
Ind. 13 – Quality, comprehensive transition planning in the IEP<br />
Ind. 14 – Post-school outcomes (working, learning, living)</p>
